Informed Consent and Acknowledgment

Please read the following consent and acknowledgment carefully. By checking the box below, you agree to the terms and conditions for your child's participation in the camp.

I hereby give my approval for my child’s participation in ANY and ALL activities prepared by UNITY AND STRENGTH INC. (AFACHO). In exchange for my child's acceptance into Unity and Strength Inc.’s camp, I assume all risk and hazards incidental to the conduct of the activities and release, absolve, and hold harmless all respective officers, agents, and representatives from ANY and ALL liability for injuries arising out of traveling to, participating in, or returning from selected camp sessions.

In case of injury, I hereby waive all claims against UNITY AND STRENGTH INC., including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. I understand that there is a risk of being injured that is inherent in all sports activities, including but not limited to the risk of fractures, paralysis, or death.

Medical Release and Authorization

Please review the following medical release and authorization. By agreeing, you provide permission for necessary medical treatment in case of an emergency involving your child.

As the parent/guardian, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional in the event of a medical emergency involving my child. I understand that this authorization is given to ensure my child receives immediate attention if required, which may include medical or minor surgical treatment, X-rays, examinations, and immunizations. This authorization is granted to prevent further endangerment to my child’s life, disfigurement, physical impairment, or undue pain and discomfort.
